I am 5'4" and I have been using this seat belt adjuster for myself for over 10 years, even in cars that have the shoulder-slider in the car to try to correct the angle. During those 10+ years of use, I've had a few minor accidents and one major one that totaled my car. Each time, the adjuster did not interfere with the proper operation of the seat belt. Some people report problems with the adjuster staying closed: the snaps are very stiff, which is a good thing. Once you put it on properly, the snaps will stay closed during daily use. You put it on near the buckle. One side snaps closed to contain the lap portion of the belt, and the other goes on the shoulder portion. Because of the strap connecting the two halves of the adjuster, the angle of the belt is narrowed as it leaves the buckle and you can correctly position the lower part over your hips and have the shoulder belt rest on your shoulder and not your neck. The belt may twist when you unbuckle and release it, so just check that it's smooth and untwisted before you buckle up again. After driving my car with it on, my 5'6" mom bought one, too.
